Advance Directives
We encourage all patients to complete an advance healthcare directive. This allows you to communicate your preferences for medical treatments and to select an agent or person to make healthcare decisions for you in case you're unable to or you want someone else to make such decisions. More information will be given to you regarding advance directives upon request.
If you already have an advance directive, please bring a copy with you when you're admitted to the hospital so you can discuss it with your healthcare team. Your advanced directive will be placed in your medical chart.
