PLEASE DESTROY ALL OLD FORMS - THEY WILL NO LONGER BE ACCEPTED BY ACCOUNTING

A. Employees submitting old forms will have those forms sent back to them with a request that they use the new form.
B. Employees submitting forms without the necessary attachments (i.e. receipts) or without all of the applicable fields completed
(i.e. miles without any destination listed) will have their form sent back to them with the reason(s) for the rejection.
ONLY COMPLETED FORMS WILL BE ACCEPTED FOR PAYMENT PROCESSING!!!!!
